Clinical Care Coordinator

The purpose of the Clinical Care Coordinator position is to provide case management and care coordination. The Clinical Care Coordinator will work with clients to determine and provide access to appropriate mental health and wellness services in a case management format. This position will work as part of an interdisciplinary wellness team consulting with mental health and wellness providers to ensure the appropriate resources and services are offered and provided. Qualified candidates must have a bachelor’s or master’s degree in social work or a related field and hold a License in Social Work (LSW) for the state of Minnesota. The Clinical Care Coordinator reports to the CEO/COO/Medical Director.

Location

This position will be located at the Northfield Secure Base Wellness Center.

Responsibilities

  • Meet with clients and families to assess the need for various services and supports

  • Schedule necessary intakes, assessments, services and appointments with appropriate providers based on provider referrals and/or client needs assessments

  • Provide ongoing support via phone or in-person meetings to connect and educate clients on support within SBCC as well as outside services

  • Consult with the interdisciplinary team to ensure appropriate services are provided for each client

  • Work closely with the billing department to determine insurance coverage of services being recommended or provided

  • Complete and submit all necessary documentation accurately and in a timely manner based on established guidelines

  • May attend interdisciplinary reviews for clients and providers

  • Ability to communicate effectively with patients, staff, physicians and the public 

  • Advanced needs assessment skills 

  • Sound knowledge of adult health and related diagnostic and clinical therapies 

  • Ability to teach patients, families, staff and students; knowledge of wellness services

  • Ability to deal with high acuity/complexity and rapidly changing situations

Pay

Secure Base Counseling Center pays employees every two weeks. Paychecks include a statement detailing the sessions being paid. Travel reimbursement, if applicable, is paid every two weeks, along with payroll, at 40 cents per minute. Reimbursement for approved expenses will also be paid every two weeks, along with payroll.

Freedom to Act

Employee is generally free to organize and prioritize own job duties, as well as assignments to other support staff, and knows where to refer people for assistance or guidance when necessary – or appropriately follows up to acquire a resolution. Employee works independently and with minimum supervision in carrying out necessary tasks. Employee is able to make decisions and answer questions using written resources such as the Employee Handbook, policy and other resources. In the rare event the Clinical Administrator is not available and the need is urgent, direct contact with the Clinical Director may be sought.
